Linked .md files/TOC

I'm writing a long book, much too long for a single .md file. Is there a way to chain .md files together, ideally from the table of contents? Example:

[TOC]
chapter1.md
chapter2.md
chapter3.md
[/TOC]

I recently tried to link to a file on disk using relative links [Mentioned Here](http://daringfireball.net/projects/markdown/syntax).  But to no avail.

I too would like to be able to link to other .md files.  I see the workflow as being similar to Tomboy Notes' Wiki-Style note taking.
